We often ask foster homes to allow prospective adopters to meet the pet prior to placement. Would you be accepting of a home visit, or would would you prefer we pick up the foster animal and take it to meet the adopter?
Please provide the names and telephone numbers of two references. These should be people who would know how you would take care of the foster animal. Also, please advise them of the potential of us contacting them.

The following conditions must be read very carefully and are non-negotiable.

I understand that Angel's Dream Small Animal Rescue Society will be responsible for any medical, training or other approved expenses associated with the foster animal in my care. Note: If injuries or medical treatment is required as a result of negligence on the part of the foster home, the foster home will be held responsible for covering the costs of treatment.INITIAL:

I also understand that by submitting this form I am agreeing to release and covenant to hold harmless Angel's Dream Small Animal Rescue Society and it's members from any claims, damages, costs or actions.

I will notify Angel's Dream Small Animal Rescue Society immediately should the foster animal become lost, stolen, ill or die.

I agree to contact my Angel's Dream Small Animal Rescue Society mentor at least once per week to update her/him on the condition and well-being of the foster animal.

Angel's Dream Small Animal Rescue Society strongly discourages training or behaviour modification through physical violence of any sort. We will not condone, nor tolerate hitting, 'spanking' or any other form of physical punishment.

All foster dogs and cats must wear a City of Calgary license tag if fostered within the city of Calgary. This ID tag must be on the animal at all times and if the foster home does not have one, the foster must contact their Angel's Dream Small Animal Rescue Society representative to request one. If the foster fails to do so they will be responsible for any bylaw fines incurred.

The foster home agrees to provide high quality food for the foster animal at their own expense. Treats, toys, beds, etc. are also the responsibility of the foster home. Angel's Dream Small Animal Rescue Society will try to help with these incidentals wherever possible.

Angel's Dream Small Animal Rescue Society reserves the right to perform unannounced visits to the foster home at any time.

Angel's Dream Small Animal Rescue Society also reserves the right to remove the animal from the foster premises at any time. Note: If Angel's Dream Small Animal Rescue Society judges the foster animal to be in danger, this initialed document releases Angel's Dream Small Animal Rescue Society to obtain the animal from said premises without warning.

INITIAL: The Foster/Representative understands that if accepting an animal into rescue as an agent of Angel's Dream Small Animal Rescue Society, that animal becomes the legal property of Angel's Dream Small Animal Rescue Society regardless. Any pulling, approved vetting fees, or adoption fees paid for out of pocket have been done so as an agent of Angel's Dream Small Animal Rescue Society and does not entitle the Foster/Representative to ownership of said animal. These expenses will be reimbursed by Angel's Dream Small Animal Rescue Society upon receipt of said animal.INITIAL:

The Foster/Representative understands that if they wish to adopt any Angel's Dream Small Animal Rescue Society foster animal, proper adoption procedure must be followed and if there is a waiting list for said animal, they will be put on the waiting list like all applicants. Special circumstances will be considered by the Angel's Dream Small Animal Rescue Society Board of Directors.

The foster home understands that Angel's Dream Small Animal Rescue Society cannot predict the behaviour of any foster animal, in any situation. Angel's Dream Small Animal Rescue Society will provide any relevant information on the foster animal's history and temperament that is known. This information may or may not be a reliable indicator of how the foster animal will behave in a new environment, including your home.

I certify that I am at least 21 years of age and that I will be solely responsible for the care and well-being of any animal that I foster on behalf of Angel's Dream Small Animal Rescue Society. Any misrepresentation of the true facts in this application will invalidate the foster home agreement and will give Angel's Dream Small Animal Rescue Society the right to immediately reclaim the foster animal.
